چکیده

A 69-year-old male cardiac transplant recipient with end-stage renal disease secondary to cyclosporin toxicity, presented with a painful swollen right brachial AVF following difficult cannulations over several consecutive dialysis sessions. On inspection there was a tender soft tissue swelling overlying the fistula, but a thrill was still palpable. Neurological examination revealed weakness of the muscles supplied by the terminal motor branch of the median nerve, with loss of sensation over the distribution of the terminal sensory branch. A duplex Doppler study demonstrated a pseudoaneurysm of the brachial artery surrounded by haematoma. In view of ongoing pain and progressive neurological signs the patient underwent emergency exploration of the AVF under general anaesthesia. A large haematoma was identified deep to the fascia and evacuated. The AVF was tied off and the patient underwent an uncomplicated post-operative recovery, with partial recovery of median nerve sensory and motor function. Dialysis continued via a tunnelled percutaneous internal jugular vascular catheter. Case 2

Thermal Resistance Anastomosis Device for the Percutaneous Creation of Arteriovenous Fistulae for Hemodialysis.

PURPOSE To evaluate the safety and efficacy of arteriovenous fistula (AVF) creation with a thermal resistance anastomosis device (TRAD). MATERIALS AND METHODS From January 2014 to March 2015, 26 patients underwent ultrasound (US)-guided percutaneous creation of proximal radial artery-to-perforating vein AVFs with a TRAD that uses heat and pressure to create a fused anastomosis. Primary endpoi...

Primary and secondary patency rates and complications of upper extremity arteriovenous fistulae created for hemodialysis.

The types of fistulae used and their complication rates are important for the hemodialysis patients. We aimed to compare retrospectively the primary and secondary patency rates and complications of upper extremity arteriovenous fistulae. Between 1984 and 2005, a total of 1,233 upper extremity arteriovenous fistulae were created in 920 patients. The mean age was 42 +/- 21 years. The fistulae wer...

Cerebrovascular manifestations in 321 cases of hereditary hemorrhagic telangiectasia.

BACKGROUND AND PURPOSE Patients with hereditary hemorrhagic telangiectasia (HHT) are at risk for developing cerebral vascular malformations and pulmonary arteriovenous fistulae. We assessed the risk of neurological dysfunction from these malformations and fistulae. METHODS Three hundred twenty-one consecutive patients with HHT seen at a single institution over a 20-year period were studied. A...

Hemodialysis Vascular Access Morbidity 1 Harold

Complications associated with hemodialysis vascular access represent one ofthe most important sources of morbidity among ESRD patients in the United States today. In this study, new data on the magnitude and growth of vascular access-related hospitalization in the United States is presented, demonstrating that the costs of this morbidity will soon exceed $1 billion per yr. This study also revie...

Arteriovenous fistulae as an alternative to central venous catheters for delivery of long-term home parenteral nutrition.

BACKGROUND & AIMS The success of home parenteral nutrition (HPN) programs is compromised by complications of central venous catheters (CVCs), such as occlusions and bloodstream infections. We performed a retrospective analysis of complication rates of arteriovenous fistulae versus CVCs in patients on long-term HPN. METHODS Data were collected from 127 consecutive patients who received HPN bet...
